Add the option to include a document (pdf, etc.) to the "New Question" submission page.

All "new question submission" forms have a "browse" button to select an attachment. Files of any types may be attached to a question.

Was looking to be able to put a pdf document on the actual submission page. The intent is the provide a document for our customers to view that explains the appropriate category to select.
